Veterinarian; submission of the declaration of renunciation of the license to practice medicine

If you no longer wish to practice the veterinary medical profession and no longer wish to use your professional title, you have the option of renouncing your license to practice as a veterinarian.

This also ends your compulsory membership of the Bavarian State Chamber of Veterinary Surgeons.

You send the original of your license to practice medicine and the completed form (see "Forms") to the responsible government.

The government of Upper Bavaria is responsible for the administrative districts of Upper Bavaria, Lower Bavaria, Upper Palatinate and Swabia; the government of Lower Franconia is responsible for the administrative districts of Upper, Middle and Lower Franconia.
