Mobile geriatric rehabilitation; application for funding to set up
The Free State of Bavaria supports the development of mobile geriatric rehabilitation in Bavaria.
Description
Purpose
With regard to the consistent implementation of the principles of "outpatient before inpatient" and "rehabilitation before care", support for the development of mobile geriatric rehabilitation in Bavaria is intended to contribute to ensuring needs-based provision of outpatient medical rehabilitation services.
Object
Funding is provided for the initial phase of interdisciplinary teams for the provision of mobile geriatric rehabilitation services (MoGeRe teams).
Recipients of funding
MoGeRe teams that have been offered the prospect of a care contract by the Association of Health Insurance Associations in Bavaria (ARGE) or with which the ARGE has concluded a care contract are eligible to apply.
Eligible expenditure
Funding can be provided for expenses that are necessarily incurred in the initial phase of a MoGeRe team and are not covered by the supply contract with the ARGE.
In particular, these are
- Personnel expenses
- Material expenses (e.g. office equipment, office technology, office supplies, technical equipment required for the rehabilitation service)
- Rent for premises of the MoGeRe team
- Public relations expenses (e.g. flyers, website, travel expenses).
The initial phase begins at the earliest from the date of the written prospect of the supply contract by the ARGE. The initial phase ends no later than 12 months after conclusion of the contract. The maximum funding period in the initial phase is 12 months.
Type and amount
The grant is provided as a subsidy of a maximum of EUR 25,000.00 per MoGeRe team. The grant is awarded by way of project funding as shortfall financing. The grant recipient must bear at least 10% of the eligible expenditure themselves. There is no legal entitlement to funding.Contact person

Essential requirements
- Written prospect of a supply contract or conclusion of a supply contract with the ARGE
- The initial phase of the MoGeRe team cannot be financed in any other way (i.e. the grant is subsidiary to all other benefits, see Article 23 of the Bavarian Budget Code (Bayerische Haushaltsordnung - BayHO)).
- Measures that have already been started cannot be funded.

Application
Applications must be submitted to the Bavarian State Ministry of Health, Care and Prevention (Department 46).
Approval
The Bavarian State Ministry of Health, Care and Prevention is responsible for approval.
Payment
The Bavarian State Ministry of Health, Care and Prevention is responsible for disbursement.
The grant is paid out upon written application.
In principle, a final installment is withheld from the grant, which is only paid out after the proof of use has been checked.
Proof of use
The recipient of the grant must submit proof of use no later than six months after the end of the funding period.
The Bavarian State Ministry of Health, Care and Prevention is responsible for checking the proof of use.
Deadlines
The maximum funding period in the initial phase of a MoGeRe team is 12 months.

Documents
- Proof of sponsorship
The legal form of the MoGeRe team must be specified exactly and proven by suitable documents (e.g. extract from the commercial register or register of associations).
- Inception of a supply contract / supply agreement
The written promise of a supply contract by the ARGE or the supply contract itself must be enclosed.
- a detailed description of the measures applied for
The application must contain a detailed description of the measures requested and the period envisaged. The measures must be economic and appropriate. The necessity of the support must be presented.
- Statement of total expenditure for the measures requested
The calculated total expenses must be presented in detail. Only expenses that are necessarily incurred in the start-up phase and that are not reimbursed by the ARGE supply contract may be claimed. Expenses that are incurred proportionately in the set-up phase may only be included proportionately.
- Financing of total expenditure
It must be shown how and by whom the total expenditure is financed (e.g. own funds [at least 10 %], loans, grants from third parties, donations, grant from the Bavarian State Ministry for Health and Care).
- Form 'Facts relevant to the subsidy ... - Declaration'.
