Surname Change Child - Declaration of Affiliation

If you and your spouse subsequently determine a married name after your child has reached the age of five, the married name will only extend to your child's maiden name if it follows the name declaration. If your child has reached the age of 14, he or she can only make the declaration himself; as a legal representative, you must agree.  

Please contact the registry office.

• birth certificate child (possibly with official translation),
• identity cards or passports,
• Proof of your name change.

In individual cases, further documents may be required. You can inquire about this at the registry office.

Your child must join the name change if they are already 5 years old.

You can submit the declaration of the name change at any registry office; it must be publicly certified The declaration becomes effective as soon as it is received by the registry office that certified the birth of your child.

You will receive a certificate of the name change.

The marriage register and the birth register are updated.

The declaration must be received by the competent registry office within three months.

Fees of 23.50 euros are charged for the certification of the declaration, unless the municipality has set a different fee by statute.

If you or your spouse or child does not speak german, you will need to use an interpreter at your own expense.
